By checking this box, I, the undersigned parent/guardian hereby release 4Ever Elite Youth Services, its staff, and partners from any and all liability for injuries, accidents or illnesses that may occur while participating in programs, events, or activities. I understand that my child participates at their own risk.

By checking this box, I grant permission for my child's image, voice, or performance to be photographed, recorded, or used in publications, social media, and promotional materials by 4Ever Elite Youth Services.
